The Usable Health™ protocols take into account your health profile, your medical data, your personal goals and preferences, and is tuned over time based on your progress and health information.

2. Creating an Account and Records—Collection of Your Personal Information

You choose whether to create an account with Usable Health™. The first time you sign in to Usable Health™, you will be asked to create an account. To do so, you must provide personal information, such as your email address, name, and date of birth. To better assess your health and wellness and to create a plan that is tailored just for you, Usable Health™ will request additional information, such as height, weight, and age. Medical record information you provide or transfer to Usable Health™ will also be used as described in this privacy policy.

By default, you are the custodian of any records you create. You can close your account at any time by signing into your Usable Health™ account and editing your account profile or by sending us an email at support@usablehealth.com.
